We have you fill out a health Questionnaire ahead of time.  You need to complete it to the best of your ability and return it to the office by fax or mail at least 24 hours prior to your appointment so that Dr. Lurie has time to review your health history before your appointment.  You then sit down with him and go over the details and other items perhaps not covered in the Questionnaire.  All aspects of your health are important so Dr. Lurie can provide the best possible wellness program for you.
